I am an in-network provider with the following insurance companies:

If you wish to use insurance for your visit, please call your insurance carrier prior to your appointment to confirm your plan benefits and to verify that I am an in-network provider with your specific plan. Please remember that ultimately your insurance benefits are a contract between yourself and your insurance company and that it is your responsibility to be aware of your benefits.

If your plan is not listed above, I am happy to provide a Superbill for you to submit to your plan to use your out-of-network benefits.

Please note I am not a participating provider with Medicaid or Medicare. Please inform me prior to your appointment if you use either of these insurance options.

• New Patient Visit (with or without Acupuncture/Bodywork) (60 Minutes): $350

• Return Patient Visit (with or without Acupuncture/Bodywork) (60 Minutes): $200

• Return Patient Brief Telehealth Check-In (30 Minutes): $150

• Blood Draws: $30

• B12 Injections: $15 (plus cost of nutrients)

• Intrauterine Insemination (fresh sperm): $550-650

• Intrauterine Insemination (frozen sperm): $350-450
