New patients: My practice is currently at capacity and I would be happy to add you to my waitlist for any availability that opens up. If you are planning or undergoing fertility care, please let me know as I prioritize scheduling for fertility patients.

Please use the form below to share a little about what you’d like support with and whether you’re hoping to use insurance (I currently accept Regence/Blue Cross Blue Shield and Moda), and I will respond to you with next steps and expectations around waitlist timeframe.

Scope of care: I am considered a specialist with insurance panels and do not serve as a primary care provider (PCP). I’m happy to be a supportive part of your care team alongside your PCP, but I do ask that patients have a PCP managing their primary and preventive care needs.
